At the direction of your physician, a nursing team consisting of a registered nurse, licensed practical nurse, and nursing assistant will provide personal, illness-specific care. The team will develop a comprehensive plan of care designed to meet your specific healthcare needs and to assist in your return home. Your RN will keep your physician informed of any particular services that are available upon your discharge.
